Case Management

The purpose of case management is to coordinate client access to services within the agency and in the community. Our team of Wellness Coordinators serve as the initial contact for clients requesting services, assist clients in connecting with primary health services and educate them about health issues and living with HIV.

Social Case Managers work with clients on a long-term basis to address dual diagnoses, mental health needs, and administer financial assistance programs for housing and health insurance.

Medical case managers ensure that clients have access to needed medical services and encourage adherence to prescribed treatment regiments. They coordinate with medical providers, provide the information, referrals and linkages needed for medical services, provide education about medical therapies and monitoring and following up on the goals of the care plan.

Whatever the need, case management strives to link clients with a solution.
