Health Services of North Texas (HSNT), a private non-profit organization, is dedicated to improving the lives of underserved North Texans and the overall health of our community. HSNT provides medical care and support for people living with HIV/AIDS, guardianship for people living with Alzheimer’s disease and developmental disabilities, and medical care to uninsured adults, children and families in North Texas.
Current Open Position
HSNT has an immediate opening for a Medical Case Manager in our Greenville, Texas location. This full time position will be responsible for overall supervision, coordination and monitoring of Case Management Services.
Primary Responsibilities will be:
•Coordinate client health care for persons with HIV or AIDS through the continuum of care; monitor and advocate for access to referrals and health care services.
•Ensure adherence to case management policies and procedures, standards of care and continuum of care to accommodate CM goals.
•Select, develop, assess, and retain case managers for Greenville location.
•Support and supervise case management team members assigned to Greenville location.
•Serve as Site leader, resolve problems encountered during daily operations and determine appropriate solutions.
•Promote a high level of customer service.
Seeking a leader who meets the required qualifications:
•Master’s degree in health or human services required
•Minimum of four (4) years progressive responsibility in utilization review/case management or social work in a healthcare setting
•Working knowledge of HIV related issues and/or infectious diseases
•Strong communication skills
•Proven conflict management skills
